When safety has its own meeting, it usually has its own silo. Risk that lives outside the operations review lives outside the decisions that create it.
Most companies run a monthly safety meeting and consider risk handled. Yet the decisions that generate risk – staffing, throughput targets, maintenance deferrals, new equipment, schedule pressure – are made in the operations review, where safety is often absent. That structural gap, not a lack of caring, is why good safety programs still underperform.
Recordable rates, claim counts, and citations are lagging indicators. They are real and they matter, but by the time they move, the exposure has already happened.
